OUR PREFERRED METHOD OF RECEIVING APPLICATIONS IS ONLINE.


There is a link at the bottom of the page that will take you to our online application.


If it is not possible for an online application to be completed, we are leaving paper applications in the

mailbox outside the front door at our office on Court Avenue.

Online and paper applications both require signatures from all adults

in the household who over 18 years old.

​

If you are a homeowner seeking assistance, contact your mortgage first and make sure they allow

third party, partial payments.

​

The following documents are required to apply for rental or mortgage assistance:

​

A letter from landlord stating the amount of rent you owe, that they are willing to work with our office, and your landlord's name, address and phone number.

​

The first and last page (with signatures) of your lease.

​

An itemized statement with all banking activity for the past 30 days.

 

Proof of income for the past 30 days — including pay stubs, social security statement, and

documentation of any income received within the last month.

​

Copy of child support — it is Indiana law that all applicants apply at the Clark County Prosecutor's Office

if not receiving court appointed child support payments.

Notice of benefits from [FSSA] Family and Social Services Administration (if receiving)

​

Depending on case, our office might ask for additional documentation. EX. RECEIPTS, DOCTOR'S NOTES, etc.

APPLICATIONS  ARE TO BE FILLED OUT COMPLETELY AND SIGNED ON LAST PAGES.

ALL FAMILY MEMBERS IN HOUSEHOLD WHO ARE 18 YEARS OR OLDER MUST FILL OUT AND SIGN APPLICATION AS "OTHER ADULT."

 

IF YOU ARE CURRENTLY UNEMPLOYED, STATE LAW REQUIRES THAT YOU ARE ACTIVELY SEEKING EMPLOYMENT IN ORDER TO RECEIVE TRUSTEE ASSISTANCE.

OUR OFFICE WILL REQUIRE A COMPLETED JOB SEARCH FORM, IN ORDER TO ASSIST.
